Why is foreach not working?

That's probably because all you need to do is return $val you do not need this:

$test ="";
  foreach ($query->result_array() as $row)
  {
     $test .= $row['id'];
     $test .= $row['name'];
  }
  return $test;

Should be:

$val = $query->row_array();
return $val;
