found the problem

it was the admin/index.php a simple typo
header("location:".SITE_URL."admin/includes/index?p=dashboard");

changed to
header("location:".SITE_URL."admin/includes/index.php?p=dashboard");

took forever to find this problem on my own.

thanks to all those who did take the time to respond.
