ASmallOrange - I'm on their Tiny plan which is a cool $25/yr. I've had some minor issues (servers reconfiguring themselves, downtime) but their customer support is excellent. If you need SSH, a special package installed, whatever - submit a trouble ticket and within the hour it will be done.

I have used the following hosting providers:

Linode for VPS service.

A2Hosting for shared hosting and reseller account, great support service.

I also used Lunarpages and Hostmonster (the same service, different prices, not sure why).
