Problem with the dropdown listing PHP

hi guys,
i have a tiny problem that is
there is a form in that
name , ip address and so on boxes are there and 2 drop down list also placed there.my problem is when i made the full site the upper drop down list values comes to the 2nd.but each values are go to each drop down.but its not happening.the of each are come from each table values are comes from the database.and go to 1 particular table
give me a solution for it.
