Question about Freak Auth Registration and Activation

I registered as a user. I received an email requesting authentication like below:

When I visit the page I get the following error on my site:

We are sorry, but your activation has failed. Contact the site administrator for assistance.

I do notice in the fa_user_temp table the newly registered user is there. All the fields are properly filled in except the activation_code field is blank.
