Images not showing when generating PDF - Please Help!!!

Hi there...

I am currently trying to generate PDF documents in a loop with DomPDF. My problem is that I have a header image that only gets inserted into the first PDF and the rest of them in the same loop are left blank?!

I would highly appreciate it if anyone can give me their thoughts on this...
