Shopping Cart (Multiple Items w/ Same Sku)

I was wondering how some of you guru's might handle adding 2 items to the Shopping Cart Class but both items have the same sku. The difference in the items being options. As of now it seems the cart class will ignore adding a second item w/ the same sku as it expects you to just update the quantity of the first item.

Ideas on handling this?

At the moment i'm forced to generate a dynamic sku, the main sku & then append additional text to the sky based on the options selected. This seems like a good solution but it goes against the product item numbering current in place by the client.

Options sometimes have some suffix on them.
So t-shirts that come in multiple sizes for example.
The base SKU is TSH
but a medium might be TSHM

Options I would say are stored separately from their base items as well which can be applied to any item that needs it.
