Take a look at revised code above. Just for ensure, save that class into 'MY_Upload.php' and put it under 'application/libraries'.

As i write at the top of previous example code, that class wasnt tested yet. But after check it, just need to add one function 'initialize()' and configure proper constructor function to make it work.
