A few questions for an ad network system

Hi,
I have a few questions for an ad network system we are creating:

The publisher (owner of a website) can create an account and request different ads (with different sizes) lets say for example user account_id: 44 has requested a 300x250 ad space.

We give him a javascript code where he places on his site and starts getting advertisements on that.

My questions:

1. How can we stop the user from using the same code over and over on the same page, so that someone does not place a code 10,000 times on a page, load the page and generate 10,000 impressions on different ads, using JS.

2. What would be a solution to immediately stop our code to work on a website that is being attacked (ddos attack for example) generally our boxes will be on hundreds of sites, if one of them gets hacked or attacked our box will also be influenced, therefore our servers, therefore our other publishers.
I am looking for a way that we blacklist the IP of a server that is being attacked (automatically).
