Code Critique

I'm hoping this is where I can do a code critique. If not I'd like to request it to be moved to the board that its location is.

I wanted to show my user management controller as well as my model that I use for this controller and see if there is any places that can be worked on. This works however it somehow feels as if the ordering logic is wrong in the controller and in the model I'm shaky if I can combine or make better use of my functions.
