With the examples you find, you should make the your code yourself, even if you are a beginner programmer. If can't do it, you should put it on e-lance, someone will do it for you for a couple $.

First one uses Jquery, and plugin uploadify

Second one does not use Jquery at all

They both come included in the "examples" download package, and should work out of the box, without any configuration or changes at all, but you will need set the upload directory setting and other customizations.
