It really looks like you need to simplify the issue and verify that your code is actually taking the path you expect. The best thing you could do is to setup a debugger and work with that to determine the difference between the working page and the pages which don't work.

If you don't want to work with a debugger, or the problem isn't obvious in the debugger, you may want to comment out some of the code (in the controller and the view) and determine whether the form validation works with one field on the view. If it does work with a single field, you can expand it until you find the field (or fields) that breaks the validation and work from there. If it doesn't work with a single field, maybe there's something going on with the controller itself, like an inheritance issue (e.g. extended the wrong controller or forgot to call the parent's method in an overridden method...).

mwhitney is right. With a debugger it's a lot easier to find bugs.

You can also use log_message to write to the log file to help you trace the execution and output variables.
