Right one thing to do is elimite parts of the problem. So first what I would do is get a product 'wordpress' or something that is guaranteed to work and needs cookies.
If it works then we know the server is setup correct. Then you know the issue is to do with the way CI is setup or something.
Also get rid of the htaccess stuff until the cookies work. Also upgrade to the CI 1.6.1. Try those and get back to us.
