As I said previously, I know almost nothing about jQuery, so it's unlikely I'd be able to re-work this example, since the values in the example you provided are being placed into a list outside of the field — I wouldn't know how to make those values pass along with the form data into the database.

If your code provides a solution to this, I'd be keen to see how that works.
