Example a):
if u use sub folder called(blaze) and u server called "server.com" need to be
RewriteBase /blaze/
RewriteRule ^(.*)$ http://server.com/$1/ [R=301,L]

Or check on the u database table called "users" password field must be
"730e59c62bc884a401c0a61480e69ae074708da4"
