I've checked it and I'm gonna try to explain it a bit better.

They have 2 important tables in this respect:
- phpbb_profile_fields: this keeps the information about a field (what otherwise would be a mysql column in the users table)
- phpbb_profile_fields_data: this holds that field's content for each user (one row = one user)

So e.g. each row in phpbb_profile_fields has "field_name", all those field_names become the header row in phpbb_profile_fields_data...

This allows great extensibility. In a control panel, you can make a form to decide which profile data you would like to get from your users, no need to "hardcode" that in you mysql database...
If you add another profile field while users are already registered, they just get blank values for those fields, no problems there.
