1366 Database error!!! Weird!!

I have a little project working in CodeIgniter the project was working properly under my test/develop computer under Windows XP, Apache. Suddenly the form in which I capture visitors data (name, email, etc) when I record the data into database I receive the 1366 error when any field had accents,ñ, etc... My database had utf-8 charset by default, my form had utf-8 accept-charset, my form page had charset utf-8, etc etc. Everything was working fine until today.

By the way I have mysql database 5.5 on my WinXP computer, PHP 5, Apache 2.2, CI 2

The same project uploaded to my Hostgator server works perfectly without any change....

The error is in DB_driver.php line 330.

on one of the pages of RTI Activist there is a error showing in google webmaster tools error is 404 not found its a forum page which is not created how to remove it.
