User Created Forms

I am new to PHP/CI, I want to be able to have my Super User create a form with whatever fields they want and then assign that form to a "Program" that I have created.   Here is an example.   I have a "Program" called Auto Insurance.   When the user chooses that program I want them to have a premade form that will include Year, Make and Model.
As a Super User, they create the programs and then I would like them to be able to also create a form and assign it to that program because each program may contain different requirements for fields.
