I am sorry if this has been solved in the past but I thought that I would offer it up.
I am using a:
- 1and1 dedicated Server.
- Codeigniter 1.7.1
I noticed with 1and1 that it requires a ? after the index.php
Notable config information.
config.php

The main difference is in the RewriteRule
On most of my other hosts this is what I use.

On 1and1 I had to add the ? at the end of index.php
