truuble with insert form data to database

Hi, all.
I have a code in my model
I call function addDisloc($city, $street, $num, $lat, $lon) in my controller(U can see a part of my controller's code): controller's code
The form html form where I send data via ajax function.
Id="flag" -- is where must be showed the result(sush as "id" of table row).
The problem in that when I insert into Myqsl database fields data from my form -- it's wery wel inserts at first time, but if I send via my form another name of the street, in database inserts only the street name. The another data is correctly inserts into a database table only when I reload the web-page and send the data with street name susch as like I sett in the previous request.
If somebody can to help whith my trouble, please answer.
