Multi-language site - howto sample

How is a multilanguage site done. I mean when all the text is stored in database and there is lang_id somehow integrated in code

how is lang_id displayed on the public views and design - as i understand every word of text becomes a variable that is translated ?

how do you integrate language_id with the view, controller, model - can you give some simple sample
