Custom Domains

I've seen quite a few web applications allow this functionality and I'm curious as to how it's accomplished on the server side. Each account would be a sub-domain and the custom domain would be mapped as a CNAME to the sub-domain of my app. What else needs to happen to get this to resolve properly?

I know how to create wildcard sub-domains, I'm trying to allow customers to use custom domains the same way Shopify does it. I think I have it figured out, though.
